Urban Health Centre 2.0: Integrated health and social care pathways, early detection of frailty, management of polypharmacy and prevention of falls for active and healthy ageing in European cities.
Urban Health Centres 2.0 (UHC2.0) promote innovative integrated health and social care pathways, early detection of frailty, management of polypharmacy and prevention of falls for active and healthy ageing in European cities. The general objective is to develop, implement, and evaluate the UHC2.0 in 5 varied European cities that will act as pilot site, in order to produce and disseminate a transferable and easily implementable model for the innovative UHC2.0. The UHC2.0 should be a platform for the realisation of the priorities of the European Innovation Partnership on Active and Healthy Ageing. In the whole project, a strong communication and optimal dissemination strategy will be realised.
To implement and evaluate the innovative UHC2.0 providing integrated care in 5 EU cities: Rijeka (Croatia), Rotterdam (the Netherlands), Valencia (Spain), Pallini (Greece) and Manchester (UK).
The program in Municipality of Pallini starts with screening for frailty during a home visit by a nurse/assistant this includes a check on available (non-prescription) medication, drug use practices, and a home safety assessment. Then, the Municipality of Pallini provides to the selected patients a tablet with a specialized application which monitors the compliance to Medication and send all these data on the cloud, where doctor has access to review the results and motivate the patiens via usefull notifications
